Set against the misty, atmospheric backdrop of Tholpetty in Wayanad, the story follows Sethu (Kunchacko Boban), a timid and mild-mannered employee at a local government health centre. Sethu’s world revolves entirely around caring for his bedridden elder brother, Madhu (Dileesh Pothan), who suffers from mental health struggles and frequently hallucinates the presence of their deceased uncle.
The mysterious circumstances of the title kick in when an enigmatic stranger, Rajendra Prasad (Sajin Gopu), enters their home. Suspected of being a Maoist, Rajendra’s presence disrupts Sethu’s quiet routine, pulling him into a web of suspicion, political tension, and escalating chaos. Sethu is forced to navigate this perilous situation while balancing his brother's delusions, creating a heart-wrenching and humorous narrative.
Joining Kunchacko, Dileesh and Sajin on the cast is Chidambaram, the director of the hit film Manjummel Boys. The film’s cinematography by Arjun Sethu was a highlight, having captured the beauty of Wayanad, while Dawn Vincent’s score also received praise.
