Set in Madurai of the 1990’s, a time in which remaining a closeted queer person was the safer and saner option, Dorothy revolves around two friends, Karthi and Subbu, played by Rishikanth and Sananth, respectively. The film explores gender identity, transgender acceptance and the rigid ideas of masculinity prevalent in a rural society.

Subbu starts of as a confident young man who grows up believing that men must be strong, fearless and never show vulnerability, only to later struggle with his gender identity and finally choose to live as a woman. This transformation becomes the central emotional conflict of the film, particularly for Karthi, her childhood friend, who initially struggles to accept the person Subbu has become.
